In this collaborative research between Hospital Clínico Universitario de Valencia, Hospital La Fe de Valencia and Quibim, we have developed highly accurate predictive models for CAR-T therapy outcomes in B-cell lymphoma patients. By integrating advanced imaging features extracted from [18F]FDG PET/CT scans with clinical data, our models can accurately predict: ▪️ Occurrence of neurotoxicity (ICANS) with an AUC of 0.830 ▪️ CAR-T response at 3 and 6 months (AUC: 0.754 and 0.818, respectively) ▪️ Overall survival and progression-free survival Notably, total metabolic tumor volume (MTVtotal) emerged as a powerful predictor, effectively stratifying patients. Those with high MTVtotal had a median predicted overall survival of 4.73 months, compared to 37.55 months for those with low MTVtotal. This innovative approach, combining imaging features and clinical variables, consistently outperformed models using either dataset alone, offering a promising tool for personalized treatment strategies and improved risk assessment in CAR-T cell therapy.
