Phone screen by HR. Technical screen by regional (or higher). Plant manager phone screen. Onsite interview (behavior based, no technical questions). I did not get the offer, and it was obvious once I met the onsite panel that I would not move along in the process. If the GP is having issues hiring people who cannot do the work, perhaps they should alter the behavior based interview portion. It is fine for some positions, but others are forced into making conflict and responses greater than reality.
