Poor management; lack of opportunities for professional growth. Leaders lack experience and do not know what's really happening on the floor so they never advocate for staff.
Staff are overworked and too little vacation is allowed (only 2 weeks! You have to beg for more than that, but 99% of the time, it's rejected.) Nurse-patient ration goes up to 1:6 and management won't do anything about it, that's why there is very high turnover rate. They always cancel staff if census is low to maximize the nurse-patient ratio. Favoritism is rampant.
